Pesticide Use Notification

By Ohio law, any parent/guardian can request notification of any pesticide treatment occurring in his/her child's school. If you would like to request notification, please email Monisa Jones at [email protected] or call 937-221-9405. 
 

Wellness Policy

 

Vision & Hearing Screening Notification

 
The Ohio Department of Health requires schools to provide annual vision and hearing screenings. These screenings help identify children who may have, or may be at risk for, vision or hearing difficulties.
 

Who Will Be Screened?

• All new students
• Vision: Grades K, 1, 3, 5, 7
• Students with incomplete follow-up from the previous school year
 

Why Screenings Matter

• Detect early vision or hearing concerns
• Support speech, language, reading, and learning development
• Identify if further testing may be needed
• Monitor changes, even if your child has passed in the past
 

Vision Screening Includes

• Observation (all grades)
• Monocular distance acuity (all grades)
• Ocular muscle balance (Grade 1)
• Stereopsis test (Kindergarten & Grade 1)
• Color deficit test (males only in Kindergarten or Grade 1)
 

Hearing Screening Includes

• Audiometry: checks hearing acuity
 

Results

• Pass: No notice will be sent.
• Fail: You will be contacted with results and referral paperwork.
 

If Your Child Fails

• Screenings are not a substitute for full exams.
• Arrange follow-up with an eye specialist (optometrist/ophthalmologist) or hearing specialist (audiologist/ENT).
• Provide the referral paperwork to the specialist and return completed forms to the school nurse.
• Need help finding specialists or financial assistance? Contact your building nurse.
• To opt out, submit a written request to your child’s nurse.
